The Certified Recovery Support Specialist (CRSS) provides experience, education and professional services to assist and support individuals in developing and/or maintaining recovery-oriented, wellness-focused lifestyles. This role involves ensuring patients receive comprehensive services, including identification into services, assessment, discharge planning, and warm handoffs. The CRSS will provide advocacy, education, recovery and wellness support, and is capable of working with patients in crisis, identifying their needs, and applying evidence-based approaches. They will engage patients in the living room process, establish collaborative relationships, and provide practical support for entering and adhering to discharge planning. The role includes assisting in the creation of recovery plans, facilitating discharge planning with accurate EMR documentation, and ensuring patients receive support and motivation for ongoing treatment. Patient care is to be provided with respect for inherent dignity, individuality, culture, and beliefs, in a compassionate, humane, and non-enabling manner. The CRSS will participate in interdisciplinary clinical staffing or ensure pertinent information is provided to the team, demonstrating Motivational Interviewing skills. They will provide continuum of care/follow-up care, be available after discharge for recovery support as needed, and bridge care with insurance-correlated community services. This includes providing follow-up instructions, appointments, and coordination for discharge planning, as well as detailed communication to discharge providers for warm handoffs. The CRSS will identify and assist with transportation barriers to appointments and work with insurance-provided transportation programs. They will provide ongoing recovery support to ensure patient engagement and adherence to treatment, helping patients navigate stages of change and practical considerations. The role requires knowledge of addiction, signs of overdose, family dynamics, and naloxone administration, including educating patients and families on opiate use and naloxone. Consultation with patients, families, insurance providers, and treatment providers to confirm coverage and affordability of treatment is also a responsibility. Collaboration with the Care Team for documentation supporting warm handoffs, assisting with EMR documentation for quality initiatives, and assisting with audits for medical necessity documents are included. The position requires working knowledge of computer operating systems, participation in in-service and competency training, meeting compliance standards for all documentation, and adhering to all Company and Department Policies and Procedures.
