About The Position
As a Pharmacy Technician – Care Manager, you’ll play a key role in helping patients understand, access, and stay on track with their medications. You’ll work side-by-side with care teams to close gaps, prevent issues, and make sure every patient gets the full benefit of their treatment. You’ll be at the center of medication care—supporting both patients and providers.
Requirements
- An active Pharmacy Technician certification (PTCB or NHA)
- Experience in a pharmacy or healthcare setting
- Familiarity with medication therapy management and chronic care support
- Confidence using EHR systems and pharmacy tools

Responsibilities
- Review and reconcile medications to ensure accuracy across care plans
- Talk with patients about how to take their medications safely and effectively
- Check in with patients who may be struggling with adherence
- Help coordinate refills, prior authorizations, and medication delivery
- Keep patient records accurate and up to date in EHR systems
- Partner with pharmacists to flag interactions and improve therapy plans
- Follow all pharmacy regulations and patient privacy standards
- Contribute ideas to improve care quality and workflows
