JOB SUMMARY: Community-based role focused on the Child and Adolescent populations. Outcomes of these services include increased rehabilitative skills-building, improved family functioning, greater access to wellness-promoting resources, and enhanced emotional and functional improvement within the family. As a part of the System of Care grant from DBHDD, the CPS-P will provide services that consist of rehabilitative skills building, the development of environmental supports and resources coordination considered essential to assist a family in improving functioning, gaining access to necessary in creating environments that promote recovery and support of the emotional and functional improvement of the family. The CPS-P will also participate in the engagement and coordination of care of children, adolescents, and young adults with other systems and internal providers.
