About The Position

This position serves as a member of the Critical Time Intervention (CTI) Team, providing supportive counseling and time-limited intensive care coordination for a caseload of high-risk clients. The role involves promoting the development of skills for coping with and managing behavioral health symptoms, providing linkage to resources, and engaging individuals in skill building. The Certified Peer Specialist will act as a role model, educate clients on self-help techniques, teach coping strategies and symptom management skills, and assist clients in developing community support systems. They will also identify and secure access to needed healthcare, social services, and community resources, collaborating with other care team members and providers to achieve optimal health outcomes. The position utilizes evidence-based practices like motivational interviewing and ensures follow-up and continuity of care. Responsibilities also include engaging clients at hospital bedside, conducting intake assessments, developing WRAP and Relapse Prevention Plans, and performing home visits to assess living conditions. Documentation of all interventions in the EHR is required.

Requirements

  • Lived experience as a consumer of mental health, substance use, and/or homeless services is required.
  • Lived experience receiving mental health services and a commitment to assist others in attaining recovery and wellness goals.
  • High School diploma or G.E.D.
  • Good written and verbal communication skills in English.
  • Basic computer and data entry skills.
  • Must be a Certified Peer Specialist with New York State.

Responsibilities

  • Engaging clients at hospital bedside to provide program overview and conduct intake assessments.
  • Developing rapport with clients to engage them in improving their health and wellness.
  • Participating in weekly team meetings to discuss new referrals and provide updates on enrolled clients.
  • Developing WRAP and Relapse Prevention Plans to help clients understand their patterns and triggers for use and identify interventions to deal with triggers and cravings and identify supports.
  • Acting as a role model to educate client about self-help techniques and self-help group processes, teaches effective coping strategies and teaches symptom management skills.
  • Providing transportation training and escorts to appointments.
  • Assisting clients in the development of community support systems and networks.
  • Providing hands-on assistance with activities of daily living.
  • Providing training in self-advocacy.
  • Supporting client goals and serving as an advocate on client’s behalf.
  • Identifying, facilitating and securing access to needed healthcare, social services benefits, and community resources.
  • Collaborating with other care team members and care providers to help client achieve optimal health outcomes.
  • Embracing the team model by collaborating with members of the team and providing support as needed.
  • Utilizing evidenced based practices, such as motivational interviewing, to empower clients to grow and attain goals.
  • Implementing tasks outlined on the care plan and ensuring follow up and continuity of care between client interactions.
  • Documenting all interventions and attempted contacts in the EHR in accordance with program standards.
  • Assessing domiciled client’s living conditions by conducting home visits.
  • Attending in-service training as requested.
  • Performing duties as assigned by supervisor.
