About The Position

In this role, you will partner with patients throughout their plan of care to restore function, improve mobility, and promote safe independence in the home. You will help maximize quality of life through prevention, treatment, intervention, and rehabilitation — all under the direction of a licensed Occupational Therapist and in collaboration with physicians, patients, and caregivers.

Requirements

  • Current OTA license in the state of Florida

  • Home Health experience preferred
  • Strong clinical judgment and independence

Responsibilities

  • Follow established agency policies and procedures
  • Assist patients in improving or minimizing physical limitations
  • Support patients in returning to their highest level of functional independence
  • Collaborate with interdisciplinary care teams
  • Participate in patient care planning and conferences
  • Perform skilled treatments as ordered by physicians
  • Monitor patient progress and communicate changes in condition
  • Educate patients and families on home programs and daily living activities
  • Complete accurate clinical documentation and progress notes
  • Assist in developing and updating plans of care
  • Complete documentation per agency timelines
