About The Position

As a Certified Occupational Therapy Assistant (COTA) in home health, you will help patients achieve their goals of safely getting dressed, preparing meals, showering independently, and returning to their normal routines within their own homes. This role offers the opportunity to treat patients one-on-one, build lasting relationships, and enjoy greater flexibility and autonomy compared to a traditional rehabilitation setting. You will work alongside experienced Occupational Therapists and an interdisciplinary team to provide exceptional care.

Requirements

  • Active Minnesota Certified Occupational Therapy Assistant (COTA) license.
  • Passion for helping patients maximize independence and quality of life.
  • Strong communication and relationship-building skills.
  • Valid driver's license and reliable transportation.

Nice To Haves

  • Home health experience is welcomed but not required.

Responsibilities

  • Carry out individualized treatment plans established by the supervising Occupational Therapist.
  • Help patients improve independence with activities of daily living, mobility, coordination, and upper extremity function.
  • Reinforce home exercise programs and educate patients and caregivers on safety strategies.
  • Recommend and educate patients on adaptive techniques and equipment.
  • Collaborate with physicians, nurses, therapists, and the interdisciplinary care team.
  • Complete timely and accurate clinical documentation.
