About The Position
Join our team at American River Home Care as a Certified Occupational Therapy Assistant (COTA) in the role of Rehab Care Coordinator. Your main responsibilities will involve supporting therapy services' daily operations, including coordinating home health care visits and managing administrative tasks to ensure a seamless care delivery process. Additionally, you will be tasked with providing direct patient care as needed.
Requirements
- Completion of an accredited Occupational Therapy Assistant program.
- Possession of current certification or licensure as mandated by state regulations.
- Maintenance of current CPR certification.
- Excellent communication, organizational, and problem-solving capabilities.
- Possession of a valid driver's license, access to an insured vehicle meeting state/organizational standards, and in good working condition.
- Demonstration of a compassionate attitude, empathy towards patients, and a strong commitment to delivering high-quality care.
Responsibilities
- Collaborate with the Clinical Therapy Manager to handle daily office tasks like reviewing referrals, conducting patient care audits, scheduling appointments, managing patient records, and coordinating communication with healthcare providers.
- Manage and schedule home health care visits to guarantee the timely and efficient delivery of therapy services to patients.
- Serve as a primary point of contact for patients, caregivers, and healthcare professionals, addressing inquiries and organizing care plans.
- Assist therapy staff by preparing necessary documentation, tracking visit schedules, and ensuring compliance with regulatory standards.
- Work with the therapy team to improve patient outcomes, streamline processes, and enhance the overall quality of care.
- Engage in quality improvement initiatives to contribute to the ongoing enhancement of therapy operations.
- Safeguard patient information confidentiality and maintain ethical and professional standards in all interactions.
- Offer direct occupational therapy as per the therapist and physician's care plan.
- Participate in public relations and community activities to bolster the organization's healthcare delivery role.
- Undertake additional duties and responsibilities as delegated, adapting to departmental needs to ensure top-quality service delivery.
