About The Position

Certified Occupational Therapy Assistant (COTA) in Skilled Nursing Facility. Flagstar Rehab is now staffing a Certified Occupational Therapy Assistant (COTA) for their Rehab Department. COTAs work with a diverse and multi-disciplinary team to perform treatments and patient/family education to improve the physicality and cognition of their patients and positively impact their ability to function in their daily lives. Requirements

• Current NYS OTA License and Registration
• EMR Competence
• State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)
• Valid Proof of Identity and Work Authorization
• Malpractice/Liability Insurance (if applicable)

Nice To Haves

• Proficiency in Rehab Optima + Point Click a plus!
• BILINGUAL English/Russian Preferred

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
