About The Position

Elderwood at Hornell is seeking a Certified Occupational Therapy Assistant (COTA) to join our skilled nursing and rehabilitation team in Hornell, NY. This is a great opportunity for a COTA looking to grow in a supportive, team-driven environment focused on high-quality patient care. The role involves providing occupational therapy treatments under the direction of a licensed Occupational Therapist (OT), assisting patients with activities of daily living (ADLs), mobility, and rehabilitation goals. The COTA will also conduct individual and group therapy sessions, document patient progress, and collaborate with various teams to help patients improve independence and quality of life.

Requirements

  • Current New York State COTA license required
  • Experience in skilled nursing, rehab, or long-term care preferred
  • Strong communication and teamwork skills
  • Passion for working with geriatric / elderly populations

  • Ability to read, write, speak, and understand the English language at an intermediate or more advanced level.

Responsibilities

  • Provide occupational therapy treatments under the direction of a licensed Occupational Therapist (OT)
  • Assist patients with activities of daily living (ADLs), mobility, and rehabilitation goals
  • Conduct individual and group therapy sessions
  • Accurately document patient progress and treatment outcomes in the EMR system
  • Collaborate with therapy, nursing, and interdisciplinary teams
  • Help patients improve independence, strength, and overall quality of life
