About The Position

VitalCaring is seeking a Certified Occupational Therapy Assistant (COTA) for a Home Health PRN position. This role focuses on delivering care that restores independence to patients in their homes. As a COTA, you will play a crucial part in helping patients rebuild essential skills for daily living after illness or injury. This position is ideal for a COTA who excels in a field-based, one-on-one care setting and is driven by making a tangible difference in patients' lives. Requirements

Graduate of an accredited Occupational Therapy Assistant program.
Active COTA license/certification in the state of practice.
Current CPR certification.
Ability to work independently in a field-based environment.
Strong clinical observation, communication, and time management skills.
Valid driver’s license, reliable transportation, and auto insurance.

Nice To Haves

Home Health experience strongly preferred.
Experience delivering therapy in patient homes vs. facility settings.
Ability to adapt care plans to real-life home environments.
Confidence identifying and escalating changes in patient condition.
A balance of clinical skill and compassionate care.

Responsibilities

Work under the direction of a Licensed Occupational Therapist to deliver personalized, goal-driven care.
Implement treatment plans to improve patients’ ability to perform daily activities.
Provide hands-on therapy focused on strength, coordination, mobility, and self-care.
Retrain patients in ADLs (dressing, bathing, meal prep, etc.).
Assess and report on patient progress, function, and home safety needs.
Educate patients and caregivers on techniques, exercises, and safety strategies.
Identify changes in condition and communicate proactively with the OT and care team.
Complete timely, accurate documentation in the EMR.
Support a safe, effective, and coordinated plan of care.
