About The Position
TheraCare is seeking a licensed Certified Occupational Therapist Assistant (COTA) for a PRN home health opportunity providing in-home physical therapy services to adult and geriatric patients throughout Pasadena and surrounding areas. This PRN home health COTA position is ideal for clinicians looking for flexibility, autonomy, and the ability to provide one-on-one patient care in the home setting. Therapists enjoy creating their own schedules while helping patients improve mobility, strength, safety, and overall independence in the comfort of their homes.
Requirements
- Associate Degree from an accredited Occupational Therapy Assistant (COTA) program
- Active Certified Occupational Therapy Assistant (COTA) license in the state of Texas
- Home Health experience (1 year required)
- Current CPR certification
- Strong communication, clinical judgment, and time-management skills
- Valid driver’s license, reliable transportation, and current auto insurance

- HomeCareHomeBase (HCHB) experience (preferred)
Responsibilities
- Provide skilled home health occupational therapy interventions under the supervision of a licensed Occupational Therapist (OT)
- Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising OT
- Treat patients with orthopedic, neurological, cardiac, and post-surgical conditions
- Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities
- Apply manual therapy techniques and non-complex modalities including heat, cold, electrical stimulation, and other physician-ordered treatments
- Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies
- Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist
- Complete accurate and timely electronic documentation in compliance with Medicare home health regulations
- Follow infection control standards and home health safety protocols
- Participate in QAPI and agency performance improvement initiatives
