The Certified Nutritionist will be responsible for completing individual nutrition assessments and providing nutrition therapy to patients identified as diabetic, pre-diabetic, at-risk, or other nutritional-related diseases, or as requested by SCHC providers. This role will provide support to the Diabetes and Community Health departments by assisting with outreach content around nutrition, scheduling, counseling, tracking, and reporting outcomes. Patients will receive an individual nutrition plan that considers their medical and nutritional needs, laboratory results, cultural and personal preferences, finances, and personal goals. The position involves working with clinic medical providers and teams to coordinate patient care and serving as a nutritional resource for all staff by compiling information for nutrition programs. The Certified Nutritionist will make referrals to various health professionals and resources as appropriate and will assist with billing by acting as the rendering provider. Additionally, this role will teach group education sessions related to nutrition and develop nutrition lesson plans, handouts, pamphlets, and visual aids. Other duties as assigned.
