About The Position

Moments Hospice is seeking a Certified Nursing Assistant (CNA) to support patient comfort and dignity during their final days. The CNA performs personal care and daily living activities, reviews patient care plans, and assists patients with mobility, exercise, positioning, and nutrition. The role works under the direction of a Case Manager RN and collaborates with a dedicated RN Case Manager. The position is on the road and includes a fleet-car benefit option for business and personal use, with gas and insurance covered at minimal cost to the employee. A company cell phone, supportive on-call assistance, clear expectations, reasonable caseloads, and structured training are also described.

Requirements

• Certified Nursing Assistant (CNA) license.
• 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice.
• Excellent customer service skills.
• Strong written, verbal, and active-listening communication skills.
• Problem-solving skills.
• Time management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license.
• Valid auto insurance.

Nice To Haves

• Current CPR certification.

Responsibilities

• Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
