About The Position

To provide Direct Resident Care under supervision of the Charge Nurse. The Nurse Assistant follows established policies and procedures while providing and coordinating resident care with other facility personnel. This role involves providing nursing care to residents without violating Residents’ Rights and participating in Facility Education Programs as assigned. The Certified Nursing Assistant is responsible for accurately documenting incidents and/or unusual problems, participating in Resident Care conferences, and developing an individualized plan of resident care. They will provide nursing care as outlined by the charge Nurse and the resident Care Plan, documenting each shift of duty regarding care and treatment provided to the resident and their response. The CNA must report changes in residents’ conditions immediately to the Charge Nurse or Nurse Supervisor and follow facility procedures for admission, discharge, and transfer of residents. They must greet residents warmly, listen attentively to concerns, and demonstrate concern for all residents’ welfare by rendering immediate assistance to any resident in need. Dependability, adherence to dress code, and a cooperative spirit are essential. Job tasks include receiving reports, providing care, serving meals, assisting with hygiene, monitoring residents, and charting. Other duties as assigned by the Charge Nurse, Supervisor, and/or Director of Nursing Services.

Requirements

CNA licensed by State of CA
CPR & First-Aid Certification
Fire Card (Hospital Fire & Life Safety)
Must clear criminal background with DOJ and FBI
Ability to work in a healthcare setting with the senior population
Ability to lift fifty (50) pounds; Moving, lifting, assisting or transferring of patients may involve lifting of up to 100 pounds using proper mechanics and/or assistive lifting devices.
Ability to exert maximum muscle force to lift, push, pull or carry patients
Ability to use abdominal and lower back muscles over time without fatigue
Ability to stand for extended periods
Ability to bend, stoop, stretch, twist, sit, and reach
Fine motor skills

Responsibilities

Provide Direct Resident Care under supervision of the Charge Nurse.
Follow established policies and procedures while providing and coordinating resident care with other facility personnel.
Conduct Resident Care as assigned, completing assignments accurately and in a timely manner.
Conduct Resident Rounds daily and initiate corrective action as necessary and/or seek assistance of Charge Nurse, Treatment Nurse or Nursing Supervisor.
Identify safety hazards and emergency situations and initiate corrective action immediately.
Provide nursing care to residents without violating Residents’ Rights.
Participate in Facility Education Programs as assigned.
Attend all classes as assigned and complete assignments accurately and in a timely manner.
Assume personal responsibility for following facility procedures related to control of equipment and supplies within the unit.
Assume accountability for compliance to Federal, State and Local regulations within the unit assigned and within the span of control of the Nurse Assistant.
Report unusual problems or incidents to Charge Nurse or Supervisor.
Accurately document incidents and/or unusual problems according to established facility procedures.
Demonstrate consistent ability to work cooperatively with residents, Charge Nurse, Treatment Nurses, Restorative Assistants, other Nurse Assistance, Physicians, families, consultant personnel, and ancillary service providers.
Participate in Resident Care conferences and other facility meetings as assigned.
Participate in the development of an individualized plan of resident care for residents assigned.
Review Care Plan daily and perform nursing care as outlined.
Provide nursing care as outlined by the charge Nurse and the resident Care Plan.
Document in the Nurse Assistant notes each shift of duty regarding care and treatment provided to the resident and the resident’s response of lack of response to care provided.
Report changes in residents’ conditions immediately to Charge Nurse or Nurse Supervisor.
Perform basic skills as outlined in facility educational program, demonstrating knowledge and competence.
Seek assistance when confronted with a resident problem that requires special assessment.
Follow facility procedures for admission, discharge and transfer of residents and complete all documentation accurately and timely.
Greet residents by name and speak to resident and family members in a kind, thoughtful manner.
Listen to resident and family complaints attentively and report problems to Charge Nurse promptly.
Respond appropriately to the feelings of others consistently by listening attentively and taking follow up action.
Control angry feelings appropriately.
Demonstrate concern for all residents’ welfare by rendering immediate assistance to any resident in need.
Answer call lights promptly.
Work with a cooperative spirit wherever assigned.
Demonstrate warm, caring feelings about residents by responding appropriately to the needs expressed.
Come to work as scheduled and consistently demonstrate dependability.
Come to work in clean, neat uniform and consistently adhere to dress code.
Clock In, Receive report and assignment.
Give care to all residents assigned.
Serve breakfast trays and assist with feeders.
Remove all trays, pull up rails and roll beds down.
Obtain necessary linen, soiled linen container and proceed with assignment.
Before starting baths, check all residents, roll down beds, change incontinent residents and take ambulatory residents to bathroom.
Complete tub baths, showers or bed baths as scheduled.
Pay special attention to oral hygiene and care of nails and hair.
Make sure all call lights are secured within reach of residents and all bed rails are up on the beds before leaving room.
Report immediately to Charge Nurse anything unusual about residents.
Take all residents to bathroom before placing in wheel chairs.
Turn and reposition all bed and chair residents every two hours.
Place Bed pans, linens, washbasins and soiled linen in proper area.
Keep bedside and over-bed tables clean and uncluttered.
Keep side rails clean.
Make sure residents are up in wheelchairs for activities.
Take residents to dining room and assist in feeding if assigned to dining room.
Walk ambulatory residents to dining room and take them to bathroom every two hours and as requested.
Make sure all residents have fresh water.
Encourage fluids for all residents unable to take water unassisted.
Make sure all residents are offered nourishment at indicated times.
Encourage adequate fluid intake by providing every resident with ice water.
Make sure food percentage, intake and output, clinitest/ acetest and bowel and bladder records are completed before going off duty.
Check name bands on all assigned residents every day.
Report to Charge Nurse if resident has removed name band or if name bands need to be changed.
Check all special assignments and see that each assignment is completed before going off duty.
Empty and record all urinary drainage units.
Check all residents at the end of the shift and make sure they are clean, dry, comfortable, have all call lights within reach, bed rails are up (if residents are in bed) and are safe.
Chart on all residents, making sure to include a narrative note about the activity and progress of the resident that day.
Sign all notes with first initial, last name and title.
Report to CNA of on-coming shift.
