The Certified Nursing Assistant (CNA) will provide compassionate, patient-centered personal care to hospice patients in their residences, including private homes and facility settings. This role involves assisting with daily living activities such as bathing, grooming, dressing, toileting, and feeding, as well as offering emotional support and companionship. The CNA will also provide bedside vigil support during end-of-life care, assist with mobility and transfers, and safely use durable medical equipment. A key part of the role is observing and reporting changes in patient condition to the case manager or supervising nurse, accurately documenting care provided, and fostering a cooperative team environment. The position also includes providing support and education to family members and caregivers, and participating in required educational meetings and training.
