About The Position

The Certified Nursing Assistant - Home Health Care (CNA-HHC) reflects the mission, vision, and values of Northwestern Medicine (NM), adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and all other regulatory and accreditation standards. The CNA-HHC provides basic assistance with activities of daily living to patients and assists in the maintenance of a safe and clean environment under the direction and supervision of the Registered Nurse. Enforces teaching regarding end-of-life care. Participates as an active member of the interdisciplinary team.

Requirements

Current Nursing Assistant Certification
Maintains CNA certification as required by Illinois State Certification Board. (requires continuous period of 24 months service to maintain certification)

Demonstrates ability to assist patient with Activities of Daily Living (ADL's) such as bathing, feeding, hair, skin and oral care. Maintains patient environment in a clean and neat manner.
Follows standard precautions and CNS infection control policies and procedures consistently.
Reapplies oxygen masks and cannulas without changing flow as directed; orally suctions patients.
Manages and operates equipment safely and correctly, cleans equipment according to standards.
Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Protects the patients in the event of a medical/health care error and, as appropriate, assists in medical/health care error resolution.
Performs other duties as assigned within the CNA scope of practice.
Re-orientates patient, assists with ambulation/positioning and transports patients as needed.
Meets current documentation standards and policies. Provides documentation that is timely, accurate, up to date, complete and legible.
