About The Position

Jackson Memorial Long-Term Care Center is a non-profit, 180-bed skilled nursing facility licensed by the state of Florida. It is one of two skilled nursing facilities owned and operated by Miami-Dade County, and Jackson Health System to assist Jackson Memorial Hospital with the timely discharge of patients who cannot be easily discharged home or to other community nursing homes due to complex medical conditions. Jackson Memorial Term Care makes every effort possible to rehabilitate each patient and restore them to their maximum potential, within a friendly and homelike atmosphere. We adhere to the concept of “patient first” to accommodate the patient's individual needs (medical, nursing, social, psychological, spiritual, and recreational) and promote a sense of wholeness and well-being.

Requirements

  • Completion of an approved nursing assistant training program.
  • Valid Florida Nursing Assistance Certificate required. Certificate must be maintained as current in accordance with State requirements.
  • American Heart Association and any additional applicable life support certification for Healthcare Providers may be required at the unit level upon hire with at least 6 months validity and maintenance at JHS for the duration of employment.
  • AHA Basic Life Support (BLS) with 6 months validity

Responsibilities

  • Provides clinical duties as assigned by unit or hospital nursing leadership.
  • Performs active therapeutic approaches to include but are not limited to range of motion, ambulation, other ADL activities to residents of the nursing facility with specific training to do so.
  • Ensures residents have adequate supplies.
  • Maintains residents' surroundings in a clean, orderly fashion.
  • Observes and reports all unusual resident's conditions or reactions to Charge Nurse (to include residents incidents/accidents).
  • Answers call light promptly, attends to residents' needs, and reports to Charge Nurse when resident requires professional services.
  • Participates actively in intershift rounds, and assists with caring for residents' needs when indicated.
  • Transports resident to and from and assists with activities, therapies and appointments.
  • Demonstrates ability to work with and be sensitive to the needs of the elderly.
  • Specific accountability for protecting residents rights to dignity, freedom from abuse, neglect and exploitation as outlined in state and federal regulations governing long-term care.
  • Follows and performs all related unit specific policies and procedures, as detailed on the unit specific competency check-list.
  • Utilizes job and unit specific equipment as required.
  • Demonstrates behaviors of service excellence and CARE values (Compassion, Accountability, Respect and Expertise).
  • Performs all other related job duties as assigned.
