This role involves providing compassionate, patient-centered personal care to hospice patients in their residences, including private homes and facility settings. Responsibilities include assisting with daily living activities such as bathing, grooming, dressing, toileting, and feeding, as well as offering emotional support and companionship. The CNA will also provide bedside vigil support during end-of-life care, assist with ambulation and transfers, and safely use durable medical equipment. Key duties include observing and reporting changes in patient condition, accurately documenting care, fostering a cooperative team environment, and educating family members and caregivers. Participation in required educational meetings and ongoing training is also expected.
