About The Position

The purpose of this position is to provide direct care needs and services to the residents of the facility. The Certified Nursing Assistant (CNA) works under the direction of an RN and/or LPN to ensure residents receive proper care and attention. This role involves a variety of tasks focused on resident comfort, hygiene, safety, and overall well-being, while adhering to facility policies and procedures, including HIPAA guidelines and a No Lift Policy.

  • Ability to adhere to HIPAA MINIMUM NECESSARY guidelines and safeguard protected health information.
  • Ability to adhere to No Lift Policy.
  • Ability to demonstrate competency using current clinical software system.

Nice To Haves

  • May take record TPR, BP and weight according to the assignment and resident needs.
  • Notifies Charge Nurse of observations of need for nail care for residents with Diabetes.
  • May assist in admitting and discharging residents (i.e., getting settled in room, filling ice pitcher, taking vital signs, demonstrating call light).
  • May assist residents with ambulation, crutch walking, using a walker, and maneuvering wheelchairs, as indicated.
  • Changes colostomy bags, may give back rub, assists with care of dying resident and performs postmortem care.
  • Assists with restorative toileting programs, bed mobility and ADL programs as directed by Licensed Nurse.
  • Serves as a member of the Care Plan Team as assigned.

Responsibilities

  • Provide direct care needs and services to residents.
  • Clock-in and out at the designated hour and report to the nursing station promptly in appropriate uniform.
  • Receive nursing assignments and special instructions from the unit charge nurse, reviewing the Resident Care Guide for each assigned resident.
  • Receive reports from off-going employees as necessary.
  • Plan work assignments to meet resident needs, considering activities, visitors, therapy, and resident preferences.
  • Maintain resident hygiene, including bathing, hair care, oral care, nail care, shaving, and dressing.
  • Keep residents clean and dry, with special attention to skin care, and notify the Charge Nurse of any needs for nail care for residents with Diabetes.
  • Encourage residents to get out of bed daily and participate in activities when medically permissible, ensuring they are dressed in street clothes when up.
  • Keep resident rooms clean and tidy, changing linen as per routine and assignment.
  • Apply physical devices per doctor's orders, ensuring they are intact, comfortable, and applied per manufacturer's instructions, and check them during routine care rounds.
  • Correct any condition that may increase the risk of an accident.
  • Carry out reality orientation for confused or disoriented residents and engage in conversation during care and meals.
  • Answer call lights promptly and keep the call light button within reach.
  • Check on assigned residents throughout the shift.
  • Assist residents with turning and repositioning, maintaining proper body alignment.
  • Encourage residents to take fluids unless contraindicated.
  • Serve correct meal trays promptly and assist residents with meals as needed, encouraging them to go to the dining room when able.
  • Wash hands and face before and after each meal.
  • Work cooperatively with fellow workers, emphasizing teamwork.
  • Report any change in residents' condition or unusual observations to the Nurse in charge.
  • Complete special nursing assignments made by the Charge Nurse.
  • Attend planned staff meetings and in-service programs.
  • Make final rounds on residents, leaving them clean and dry.
  • Record Temperature, Pulse, Blood Pressure, Intake and Output for residents as directed.
  • Assist residents to scheduled activities and the dining room for meals.
  • Assist residents with ambulation, crutch walking, using a walker, and maneuvering wheelchairs as indicated.
  • Assist residents for naps and with toileting needs, offering bedpans and/or urinals as appropriate.
  • Assist in admitting and discharging residents.
  • Assist with preparing residents for tests, x-rays, physician and dentist visits, and physical therapy, and may transport them to necessary areas.
  • Assist with operating TV and radio as needed or requested.
  • Change colostomy bags, give back rubs, perform postmortem care, and assist nurses in giving treatments and examining residents.
  • Adhere to universal, standard, or other precautions, as appropriate, and wear gloves during bathing and incontinent care.
  • Assist with restorative toileting programs, bed mobility, and ADL programs as directed by a Licensed Nurse.
  • Serve as a member of the Care Plan Team as assigned.
  • Demonstrate competency using the current clinical software system.
  • Perform other duties as assigned by the Supervisor.
