MIDLAND CARE CONNECTION INCTopeka, KS
Onsite

About The Position

Midland Care Connection is a not for profit, community based organization dedicated to helping older adults and seriously ill individuals stay independent and supported in their homes. Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. Hospice House is a peaceful, 12 bed, in patient facility dedicated to providing compassionate, end of life care when a home setting is no longer feasible. Here, patients receive around the clock, skilled hospice care from a multidisciplinary team—nurses, social workers, chaplains, and other specialists—focused on managing pain and symptoms with dignity and respect. The environment is designed to feel like home, offering comfort not just to the patients but also to their loved ones, providing space for visitation, emotional support, and moments of peace in the final chapter of life.
