About The Position
Moments Hospice is seeking a Certified Nursing Assistant to support patient comfort and dignity. The role involves providing personal care and assisting patients with daily living activities, working under the direction of a Case Manager RN and collaborating closely with a dedicated RN Case Manager. The position offers structured training, reasonable caseloads, and on-call support in a supportive, collaborative team environment. It is described as on the road, and includes a company vehicle benefit option with gas and insurance covered for business and personal use at a minimal cost, as well as a company cell phone.
Requirements
- Certified Nursing Assistant (CNA) license.
- At least 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice.
- Excellent customer service skills.
- Strong written and verbal communication and active listening skills.
- Problem-solving skills.
- Time management skills.
- Ability to work in an electronic medical record system.
- Ability to lift 50 lbs.
- Good understanding of body mechanics.
- Strong understanding of professional boundaries.
- Valid driver's license.
- Valid auto insurance.
Nice To Haves
- Current CPR certification.
Responsibilities
- Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
- Review patient care plans.
- Assist with lifting and transferring patients.
- Assist with range of motion, positioning, exercise, and ambulation.
- Assist with dining and nutrition.
- Maintain electronic medical records.
- Work under the direction of a Case Manager RN.
