About The Position

Hope Christian Health Center (HCHC) is a nonprofit, faith-based organization dedicated to serving the Las Vegas community through compassionate, high-quality healthcare. Our services include primary care, behavioral health, community health worker programs, street outreach services, and recuperative care. The Certified Nursing Assistant (CNA) plays a vital role in supporting both patient care and administrative functions. Under the supervision of the Clinical Care Operational Supervisor, the CNA provides high-quality direct patient care. In addition to hands-on care, the CNA also assists with patient intake, documentation, and managing electronic health records, while ensuring confidentiality and HIPAA compliance. Per Diem Position – Open to Day and Night Shifts

Requirements

  • Current Basic Life Support (BLS) – required
  • Active CNA license in the State of Nevada - required
  • High School Diploma or equivalent. - required
  • Successful completion of a Certified Nursing Assistant (CNA) training program – required

  • Ability to operate general office machinery (computers, calculators, copy/fax machine, telephones, etc.)
  • Excellent communication skills, both written and verbal.
  • Demonstrated passion for community health care.
  • Proven ability to maintain integrity in a fast paced and high-demand environment.
  • Proven ability to manage performance goals.
  • Ability to interact with people from all ethnic backgrounds, ages, and lifestyles; ability to understand and respond appropriately, effectively, and sensitively to special populations.

Responsibilities

  • Assist patients with activities of daily living, such as bathing, dressing, grooming, and eating, as assigned.
  • Monitor patients’ vital signs, intake/output, weight, and height and report any changes or concerns to the nursing team.
  • Maintain a clean and organized patient environment, including making beds, tidying rooms, and upholding cleanliness and infection control standards.
  • Document all care provided on flow sheets, CNA report books, notes, and charts in an informative and descriptive manner.
  • Routinely check on patients to ensure their personal care needs are being met.
  • Participate in and receive the nursing report upon duty.
  • Perform only those nursing care procedures that fall within the CNA’s scope of practice.
  • Reposition immobile patients regularly to maintain comfort and prevent pressure ulcers.
  • Communicate effectively and collaborate as a productive member of the interdisciplinary care team.
  • Welcome patients and visitors, respond to phone and in person inquiries with professionalism and confidentiality.
  • Assist patients with intake forms and collection of personal belongings upon admission.
  • Process and upload intake documentation into the appropriate electronic systems (HMIS, eClinicalWorks).
  • Ensure accuracy and completeness of all forms, including required signatures and data entry.
  • Route patient requests and concerns to appropriate departments (clinical, social services, etc.)
  • Maintain and update internal patient tracking systems (census reports, referrals follow-ups, documentation logs).
  • Uphold all confidentiality and HIPPA compliance standards during information handling and documentation.
  • Complete shift reports and support general administrative tasks as needed.

  • Ensures compliance with all 19 Health Resources and Services Administration (HRSA) Health Center Program Requirements.
  • Actively participate in and support quality improvement activities and the clinical practice team efforts emphasize the importance of the individual patient and putting the patient’s needs first.
  • Performs all other duties as assigned.
