About The Position

STGi is seeking a Certified Nursing Assistant (CNA) to provide services at The William R. Courtney Texas State Veterans Home in Temple, TX. This facility focuses on providing compassionate and specialized care to veterans, prioritizing the best experience for residents and their families. The CNA will provide residents with routine daily nursing care and services in accordance with their assessment and care plan, as directed by supervisors.

Responsibilities

• Record all entries on flow sheets, notes, charts, etc., in an informative and descriptive manner.
• Report all changes in the resident’s condition to the Charge Nurse/Nurse Supervisor as soon as practical.
• Weigh residents in accordance with his/her plan of care and report significant weight changes.
• Measure residents’ vital signs (temperature, pulse, respirations, blood pressure), weight, intake, output, and food consumption and document in the medical record.
• Maintain residents’ water pitchers with clean, fresh water and ensure they are within easy reach.
• Prepare residents for meals, including assisting with toileting, hygiene, and positioning.
• Record resident food/fluid intake and report changes in eating habits.
• Assist residents with activities of daily living (ADLs) based on their individual needs, from minimal assistance to total dependence.
• Contribute to the development and implementation of interventions aligned with resident needs/goals and recognize standards of practice for ADLs.
• Monitor and evaluate resident responses to care plan interventions and treatments.
• Care for residents with mental and psychosocial disorders, including those with a history of trauma or PTSD.
• Assist in preparing residents for exams, tests, and procedures, and provide support or escort them to appointments.
• Assist in transporting residents to appointments, activities, and social programs.
• Assist residents with bowel and bladder functions.
• Assist residents in preparing for activities and social programs.
• Assist residents with ambulation, with or without assistive devices.
• Assist residents with bathing functions (bed bath, tub, or shower).
• Assist residents with daily dental and mouth care.
• Assist residents with dressing/undressing and personal grooming.
• Assist non-diabetic residents with nail care.
• Assist with lifting, turning, moving, positioning, and transporting residents.
• Assist with the application of nonsterile compresses and bandages/slings.
• Support residents through meaningful activities by simplifying tasks for those who have trouble following complex directions.
• Change bed linens in resident rooms.
• Check and report bowel movements and stool character.
• Routinely check and meet residents’ personal care needs according to their wishes.
• Collect specimens (urine, sputum, stools) as instructed.
• Maintain intake and output records.
• Observe and report pressure areas and skin breakdowns to prevent pressure injuries.
• Attend change of shift reports.
• Perform restorative and rehabilitative procedures as instructed.
• Provide non-pharmacological interventions for pain.
• Provide daily range of motion (ROM) exercises and record data.
• Provide eye and ear care.
• Report injuries of an unknown source, including skin tears.
• Greet residents, escort them to their rooms, and provide new resident orientation.
• Assist nurses with admission, transfer, and discharge functions.
• Introduce new residents to roommates, other residents, and personnel.
• Inventory and mark resident’s personal possessions and update inventory lists.
