Requirements
- High School diploma or GED required.
- 105 hours Certified Nursing Assistant Program as approved by the Indiana State Board of Health or equivalent as approved by the Indiana State Board of Health.
- Requires having name placed on Certified Nursing Assistant Registry at time of hire as required by the Indiana State Board of Health.
- Must be in good standing and maintain certification throughout employment.

Responsibilities
- Assists with admission, transfer and discharge of residents as instructed by Charge Nurse.
- Assist residents with tending to personal care and activities of daily living.
- Provide direct patient care as assigned by Charge Nurse.
- Provide proper documentation of resident care.
- Promptly communicate with Charge Nurse and leadership team with any changes in the resident’s condition.
