About The Position

Weekend Certified Nursing Assistant needed to assist patients with personal care, daily living activities, and ensure their health and safety. This role involves promoting resident self-direction and decision-making while adhering to hospice care standards. The position requires a high level of organization, excellent communication skills, and the ability to work effectively within a healthcare team.

Requirements

  • Knowledge of skills required to provide excellent care in accordance with the role of Certified Home Health Aide (CHHA) or Certified Nurse Aide (CNA) in a hospice environment.
  • Excellent interpersonal skills for interacting with patients and families, nurses, and employees.
  • Ability to comprehend spoken and written English, to communicate with clarity both verbally and in writing.
  • Visual acuity sufficient to read a thermometer.
  • Auditory acuity sufficient to hear and comprehend normal speaking tones, and to hear call bells with or without assistive devices.
  • Well organized to manage time and prepare care-related documentation.
  • Ability to complete a minimum of 12 hours of in-service education per year.
  • High School diploma or equivalent.
  • Current Home Health Aide Certification by the New Jersey Board of Nursing, or a Certified Nurse Aide approved by the New Jersey State Department of Health and Senior Services.

  • Prior experience working in a home care/hospice environment.
  • CNA & CHHA certifications.

Responsibilities

  • Assist patients with all aspects of personal activities of daily living and oral hygiene care.
  • Promote resident self-direction and personal decision-making while protecting resident's health and safety.
  • Safely assist patients with transfers to bed, chair, and commode, as well as ambulation using walkers and canes.
  • Safely assist bed-bound patients with appropriate range of motion exercises and required turning and positioning.
  • Demonstrate safe and effective use of a hoyer lift as instructed by the RN.
  • Perform shift-to-shift report, including patient observation, repositioning, and wet checks.
  • Utilize agency technology for efficiency, communication, and performance.
  • Perform all aspects of catheter care according to policy and procedure.
  • Prepare equipment for return to vendor and place in appropriate storage areas.
  • Stock supplies according to inventory and alert staff when supplies are low.
  • Apply ostomy bags and simple dressings as instructed and demonstrated by the RN.
  • Properly prepare rooms for new admissions, including bed zeroing, oxygen setup, and suction setup.
  • Provide skin care at the direction of the RN to prevent skin breakdown and report observations.
  • Assist patients at meal times, including setup, feeding, and tray distribution.
  • Observe patients' physical and emotional status and report any changes in condition or family situation.
  • Assist with post-mortem care as instructed by the RN.
  • Accurately complete all documentation before the end of the shift and report off to the incoming RN and Hospice Aide.
  • Safely change occupied or unoccupied beds.
  • Wash patient's personal laundry and bed linens when needed.
  • Observe and participate in Emergency Preparedness/Disaster plans.
  • Maintain appropriate infection control practices.
  • Actively participate in building/grounds maintenance and complete assigned tasks.
  • Maintain accurate temperature logs and cleaning schedules.
  • Demonstrate proper mouth and oral hygiene care.
  • Answer call bells promptly and follow through with patient requests.
  • Participate in all required in-service education programs and meetings.
  • Demonstrate commitment to work schedule by adhering to attendance standards.
  • Provide proper notification in advance for absence and tardiness.
  • Adhere to employment policies in accordance with the Employee Handbook.
  • Demonstrate compliance with the dress code policy.
  • Perform other duties as assigned.
