About The Position

We are looking for a Certified Nursing Assistant (CNA/NAR) to support our TCU/LTC residents in our Skilled Nursing Facility- Ridges Care Center! This role provides hands on care and support services for our adult residents at our spacious care center in Burnsville, MN. This patient support position includes: 24 hours every two weeks, every other weekend schedule, and day hours. Ebenezer Ridges Care Center highlights a full continuum of care campus including Transitional Care, Adult Day Programs, Intergenerational Child Care, Long Term Care, Enhanced Care and Assisted Living/Memory Care. We are located across the street from Fairview Ridges Hospital at the I-35E and I-35W split in Burnsville, MN. Requirements

Active as Nursing Assistant, Registered (NAR) on the MDH Nursing Assistant Registry by start date – OR- Individual who is hired into Ebenezer’s Nursing Assistant Course or Ebenezer’s Pre-CNA job for Nursing Assistants; will be granted a 4-month post hire to become active on the Minnesota Nursing Assistant Registry.

Responsibilities

Assists resident based on individual care plan with personal cares such as; bathing, oral hygiene, grooming, toileting, dressing, showers and responds to call lights
Nursing Assistant may per community patient care model check and record vitals
AM/PM/NOC cares; including peri care and catheter care
May turn, lift or transfer residents with or without other persons/mechanical assistance
Observes residents and reports status to appropriate staff in a clear and concise manner
Chart or document in electronic medical record system
Seeks to resolve resident and family concerns
Receives and gives report to appropriate staff during various times in work shift
Other duties such as; order and cleanliness in rooms and work areas, reports malfunctioning equipment to maintenance department, and answers phones in a timely manner
