About The Position

The Certified Nursing Assistant (CNA) for the PACE Program at Midland Care Connection supports older adults enrolled in the Program of All-Inclusive Care for the Elderly. This role involves assisting participants with daily activities such as bathing, dressing, grooming, and mobility. The CNA will also monitor vital signs, report health changes to nursing staff, assist with meals, accompany participants during activities, and provide overall support to ensure comfort, safety, and independence in both the center and participants' homes. The PACE program is designed to help older adults remain safely in their homes rather than entering nursing facilities, offering comprehensive medical care, rehabilitation, medications, transportation, social services, and supportive therapies, all coordinated by an interdisciplinary care team. Participants receive personalized care plans and access to a PACE center for social activities, meals, and clinical services, helping seniors maintain independence and dignity in their communities.
