About The Position

Moments Hospice is hiring a Certified Nursing Assistant (CNA) to support patient comfort and dignity. The CNA performs personal care and activities of daily living, assists with mobility and nutrition, reviews care plans, and maintains electronic medical records under the direction of a Case Manager RN. The role offers structured training, reasonable caseloads, on-call support, and close collaboration with a dedicated RN Case Manager. It is an on-the-road position, and a company vehicle with fuel and insurance coverage is offered for business and personal use at a minimal cost.

Requirements

• Required Certified Nursing Assistant (CNA) license.
• At least 12 months of caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital, or Hospice.
• Excellent customer service skills.
• Strong written, verbal, and active-listening communication skills.
• Problem-solving skills.
• Time-management skills.
• Ability to work in an electronic medical record system.
• Ability to lift 50 lbs.
• Good understanding of body mechanics.
• Strong understanding of professional boundaries.
• Valid driver's license.
• Valid auto insurance.

Nice To Haves

• Current CPR certification.

Responsibilities

• Perform personal care activities related to daily living, including bathing, dressing, grooming, toileting, and skin care.
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.
