About The Position

Suncrest At Brighton/Suncrest Hospice's goal is to change the expectation of hospice care by providing exceptional care and service to patients. This is achieved by allocating resources to increase staff to patient ratios, thereby increasing clinical visits while lowering clinician caseloads. Suncrest Hospice is a Community Health Accreditation Partner (CHAP) certified hospice. We are looking for individuals committed to providing the highest quality of care to patients and their families.

Requirements

• Strong commitment to improving quality of life while partnering with community organizations in patient care.
• Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and aide leadership.
• Eager to gain higher level of opportunities such as certifications and further education.
• Work closely with patients on a 1-1 level but get strong support from a patient care team.
• Able to work independently and on your own from a day-to-day basis.
• Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance.
• Certified Nursing Assistant (CNA) certification in the state(s) of practice.
• Home Health Aide (HHA) certification in the state(s) of practice.

Responsibilities

• Providing resident care to assigned residents in accordance with resident assessments, care plans and as directed by nursing management.
• Working with other licensed care staff to coordinate the proper types of care to residents.
• Strictly following all Company nursing policies and procedures as well as safety guidelines.
• Making visits to end-of-life patients on Saturdays and Sundays (Daytime hours).
• Making supply delivery and triage visits when appropriate.
