About The Position

Eddy Village Green is a unique community in the greater Capital Region and New York State, designed to replicate The Green House model. This innovative approach moves away from the traditional institutional feel of nursing facilities, offering a more home-like environment. EVG consists of 16 ranch-style houses, each caring for 12 elders. The staff, known as "shahbazim" (Certified Nursing Assistants or CNAs), receive specialized training to nurture, protect, and engage with the elders, fostering a family-like atmosphere. The community is located at 200 Village Green Drive and is accessible via CDTA bus route 182. Eddy Village Green was recognized as one of Newsweek's Top Nursing Homes for 2021, ranking #47 in NYS for excellent patient outcomes.

Requirements

  • Must be able to read, write, and speak English.
  • Must be a licensed Certified Nursing Assistant in accordance with New York State.
  • Must be able to understand English both verbally and written.
  • CPR/First Aid Certification required.
  • Culinary course including Serv Safe training required.
  • Excellent communication and interpersonal skills and a sincere desire to serve others.
  • Must be able to relay information concerning a resident's condition.
  • Must have patience, a cheerful disposition, and enthusiasm assisting Residents with various types and levels of need.
  • Must possess the ability to work professionally with other staff, residents, family members, and outside agencies/visitors.
  • Must be able to successfully complete Green House Core Training (on the job).
  • Must be willing to learn basic computer skills on a daily basis.

Responsibilities

  • Provide personal care assistance for the elders as a Certified Nurse Aide (CNA) while maintaining standards of Nursing Care.
  • Perform selected elder care while maintaining the dignity, autonomy, and decision-making of the elder, in accordance with policies, procedures, and regulations.
  • Work within a self-managed team to provide a home-like atmosphere, including cooking, light cleaning, coordinating meaningful events and activities, and advocating for elders' interests during the care planning process.
  • Work closely with members of the self-directed team in conflict resolution, staffing/scheduling, hiring decisions, quality assurance, and performance improvement.
  • Use LEAN principles in problem-solving within the self-directed work team, under the facilitation of the Manager Guide/Director of Nursing.
  • Maintain required staffing levels, including proper attendance per the self-directed work team schedule, with potential flexibility as per facility policy.
  • Fulfill one of six primary responsibilities in enhancing an environment that provides the highest level of quality of life and quality of care for elders: clinical care coordination, team coordination, scheduling coordinator, food coordinator, housekeeping coordinator, and activities coordinator.
  • Ensure proper accident/incident prevention behavior and immediately report care and safety concerns to nursing, Clinical Support Team members, and the Manager Guide.
  • Project, sustain, and nurture elders by creating and maintaining a warm home environment.
  • Actively participate as a respectful team member of the household's self-managed team by adhering to the house Code of Ethics, attending scheduled team meetings, communicating via email and shift-to-shift report, and responding to fellow Shahbaz and Manager Guide phone calls when off-duty in a timely manner.
  • Communicate and work collaboratively with the Clinical Support Team (activities, therapies, nursing, social work, dietary) to enable elders' highest level of autonomy, functionality, and quality of life.
  • Document elders' personal care, behavior, and interactions in an informative, descriptive manner and familiarize oneself with the elders' history, preferences, and care needs.
  • Ensure sanitary food handling and kitchen cleanliness as per ServSafe culinary training.
  • Adhere to infection control policies and procedures and maintain a clean environment.
  • Use good judgment and make decisions regarding elder care within the scope of practice, balancing resident rights with the optimum quality of care and quality of life.
  • Follow work assignments and/or work schedules in completing and performing assigned tasks.
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment through the house and shift.
  • Assist elders with hygiene needs (baths, mouth care, hair care, shaving, nail care, back care).
  • Assist with feeding of those elders able to chew and swallow without difficulty, employing special oral feeding techniques for elders with special needs/difficulties.
  • Measure intake and output accurately and document on worksheet and graphic sheet.
  • Check and report bowel movements and character of stools as instructed.
  • Perform routine colostomy care.
  • Answer elder calls promptly and ensure frequent checks for elders unable to call for help.
  • Weigh residents using upright, chair, and bed scales.
  • Measure and record temperatures, pulse, and respirations as instructed.
  • Maintain safety measures, including transferring or weighing elders using appropriate lifts.
  • Assist elders in using the bathroom, shower, urinal, bedpan, and commode.
  • Keep elders dry by changing gowns, clothing, and linen when wet or soiled.
  • Change bed linens, make beds, and provide extra covers as requested, ensuring linens are kept tight.
  • Ambulate elders, including those using canes and walkers, as instructed.
  • Assist in transporting elders to/from appointments, activities, and social programs.
  • Position bedfast elders correctly and comfortably, assist with turning elders, and assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
  • Perform skin assessments and notify Clinical Support Team members of any abnormalities in skin integrity.
  • Measure and document vital signs and weights, reporting any variations.
  • Apply cold packs and warm compresses.
  • Uphold the elder's Bill of Rights.
  • Document elder-related tasks in appropriate records.
  • Assist with postmortem care and the caring for personal belongings of the deceased.
  • Attend team meetings as scheduled.
  • Attend and participate in scheduled training, in-services, and educational classes as required.
  • Maintain confidentiality of all elder care information.
  • Perform other duties as assigned.
