This role involves providing compassionate, patient-centered personal care to hospice patients in their residences, including private homes and facility settings. Responsibilities include assisting with daily living activities such as bathing, grooming, dressing, toileting, and feeding, as well as offering emotional support and companionship. The CNA will also provide bedside vigil support during end-of-life care, assist with mobility and transfers, safely use durable medical equipment, and observe and report changes in patient condition to the case manager or supervising nurse. Accurate documentation of care, fostering a cooperative team environment, and providing support and education to family members and caregivers are also key aspects of the role. Participation in required educational meetings and ongoing training is expected.
