Requirements

Valid NYS Certified Nursing Assistant License (CNA) - Required
Ability to work in a team oriented work group
Proficient in various computer programs and Medical Records Documentation system

Nice To Haves

Minimum of 1 year experience working with Elders - Preferred

Responsibilities

Provide high quality, individualizes patient care in collaboration with our nursing team
Take initiative while assisting and maintaining patient care
Assist the resident in activities necessary as per individualized care planning
Transport Elders to and from activities
Perform evaluation and reporting
Provide resident and family with emotional support individualized care planning
Receive/give reports to team members/team leaders to appropriate professional staff
