About The Position

The primary purpose of this job position is to provide each of the assigned residents with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by supervisors. This role involves a variety of essential duties focused on resident care, safety, and comfort, as well as accurate record-keeping and communication within the healthcare team.

Requirements

Must possess, as a minimum, a 10th grade education.
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
Ability to write reports, business correspondence, and procedure manuals.
Ability to effectively present information and respond to questions from managers and employees.
Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
Occasionally lift and/or move up to 25 pounds.
Prolonged use of a desktop or laptop computer.
Regularly required to sit, stand; walk and talk, read or hear.
Frequent use of all office related equipment to include copier/scanner/fax, telephone, and calculator.
May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
Identify residents before administering treatments or serving meals using wristbands or photo cards.
Record all entries on flow sheets, notes, and charts in an informative and descriptive manner, using only authorized abbreviations.
Report all changes in resident condition, accidents, and incidents to the Nurse Supervisor/Charge Nurse promptly.
Maintain resident confidentiality and report any suspected or known violations of protected health information disclosure.
Report any known or suspected unauthorized attempts to access the facility’s information system.
Perform only trained nursing care procedures.
Ensure resident rooms are prepared for new admissions, including making beds and having admission kits ready.
Greet residents, escort them to their rooms, and introduce them to staff and other residents.
Make residents comfortable, inventory and mark personal possessions, and store clothing.
Assist residents with packing personal possessions for transfers or discharge.
Transport residents to new rooms or receiving areas, and assist with loading/unloading from vehicles.
Create and maintain a warm, positive, and calm environment.
Participate in regular meetings with nursing personnel to identify and correct problem areas and improve services.
Report all resident complaints and grievances.
Participate in and receive nursing reports.
Follow established policies concerning exposure to blood/body fluids.
Make beds, add extra covers as requested.
Measure and record resident vital signs (TPRs) and weights/heights as instructed.
Ensure residents who cannot call for help are checked frequently.
Answer resident calls promptly and check personal care needs.
Assist residents with identifying food arrangements and record food/fluid intake, reporting changes to the Nurse Supervisor.
Keep residents' water pitchers clean, filled, and within reach.
Perform after-meal care.
Follow work assignments and schedules.
Cooperate with inter-departmental and facility personnel.
Notify the facility when late or absent.
Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
Assist residents with daily functions such as dental care, bathing, hair combing, and dressing.
Keep residents dry by changing clothing and linens when wet or soiled.
Assist residents in preparing for medical tests and physical examinations.
Assist with lifting, turning, moving, positioning, and transporting residents.
Perform restorative and rehabilitative procedures as instructed.
Observe and report skin breakdowns to prevent pressure ulcers and report injuries of unknown source.
Observe disoriented and comatose residents and record/report data.
Perform special treatments as instructed.
Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
Use only trained equipment and report defective equipment.
Honor resident refusal of treatment requests and report them to the supervisor.
Report all allegations of resident abuse and/or misappropriation of resident property.
