A Certified Nursing Assistant (CNA) in the PACE Program supports older adults enrolled in the Program of All-Inclusive Care for the Elderly. This role involves assisting participants with daily activities such as bathing, dressing, grooming, and mobility. The CNA will also monitor vital signs, report health changes to nursing staff, assist with meals, accompany participants during activities, and provide overall support to ensure their comfort, safety, and independence, both in the center and in their homes. The PACE program aims to help older adults remain safely in their homes by providing comprehensive medical care, rehabilitation, medications, transportation, social services, and supportive therapies, all coordinated by an interdisciplinary care team. Participants receive personalized care plans and access to a PACE center for social activities, meals, and clinical services, fostering independence and dignity within their communities.
