Certified Nursing Assistant

Unity Hospice•Green Bay, WI

About The Position

The Certified Nursing Assistant (CNA) provides services and procedures that are delegated, assigned, and supervised by a Unity RN. These services and procedures are based on the RN’s comprehensive assessment of the patient’s personal care needs and are documented in the Activities of Daily Living Plan of Care. This is a community-based position in which the CNA travels to patients’ homes and various care facilities to provide individualized personal care and support. The CNA works independently in the community while maintaining communication and collaboration with the Unity RN and care team to ensure patients receive safe, compassionate, and appropriate care. During times when census is higher, you will be cross-trained into our hospice residence and may support the team by providing care to patients at our Meng facility. This flexible approach helps ensure consistent, high-quality care across all settings as patient needs fluctuate.
