About The Position

The primary purpose of this role is to provide assigned residents with routine daily nursing care and services in accordance with their assessment and care plan, and as directed by supervisors. This position carries administrative authority, responsibility, and accountability for carrying out assigned duties. Requirements

  • Must possess High School Diploma.
  • Must be a licensed Certified Nursing Assistant in accordance with laws of this state.
  • Must be able to read, write, speak, and understand the English language.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
  • Must be a supportive team member, contribute to and be an example of team work and team concept.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Must possess the ability and willingness to work harmoniously with other personnel.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to relate information concerning a resident’s condition.
  • Must not pose a direct threat to the health and safety of other individuals in the workplace.
  • Must be able to move intermittently throughout the workday.
  • Must be able to speak and write the English language in an understandable manner.
  • Must be able to cope with the mental and emotional stress of the position.
  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately.
  • Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
  • Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
  • Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
  • May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

  • Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
  • Identify residents using wristbands or photo ID before administering treatments or serving meals.
  • Record all entries on flow sheets, notes, and charts in a descriptive and informative manner using authorized abbreviations.
  • Report changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
  • Report all observed accidents and incidents on the shift they occur.
  • Maintain confidentiality of resident and facility information, reporting any suspected violations.
  • Ensure resident rooms are prepared for new admissions.
  • Greet and escort residents to their rooms, introducing them to others as appropriate.
  • Assist residents with personal care functions including bathing, dressing, grooming, and oral hygiene.
  • Assist residents with bowel and bladder functions, maintaining intake and output records.
  • Collect specimens (urine, sputum, stools) as instructed.
  • Assist residents in preparing for medical tests, appointments, and social programs.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Assist residents to walk with or without self-help devices.
  • Perform restorative and rehabilitative procedures as instructed.
  • Change dressings, bandages, and binders as instructed.
  • Provide eye and ear care, and assist with compresses.
  • Weigh and measure residents, and record vital signs (TPRs) as instructed.
  • Respond promptly to resident calls and check on residents who cannot call for help.
  • Assist with the care of dying residents and provide post-mortem care.
  • Observe and report skin breakdowns and pressure areas to prevent bedsores.
  • Provide daily indwelling catheter and perineal care.
  • Perform nonsterile vaginal irrigation/douches.
  • Assist with the application of slings, elastic bandages, and binders.
  • Give tepid sponge baths and provide daily Range of Motion Exercises.
  • Turn bedfast residents at least every two hours.
  • Perform diabetic urine testing and provide physical and respiratory therapy as instructed.
  • Observe disoriented and comatose residents, recording and reporting data.
  • Turn over any found medications to the Nurse Supervisor/Charge Nurse.
  • Provide Reality Orientation as instructed.
  • Monitor and report changes in room temperature, ventilation, and lighting.
  • Prepare residents for meals, assist with feeding, and record food/fluid intake.
  • Serve between-meal and bedtime snacks.
  • Perform after-meal care for residents.
  • Check rooms for food articles and unauthorized items.
  • Attend and participate in scheduled training, educational classes, and orientation programs.
  • Participate in facility in-service training programs.
  • Wash hands before and after providing resident services.
  • Keep the nurses’ call system within reach and report any resident leaving/missing.
  • Follow established safety precautions and report hazardous conditions.
  • Keep floors dry, report spills, and store supplies and equipment properly.
  • Clean, disinfect, and return resident care equipment to storage.
  • Perform routine housekeeping duties related to nursing care.
  • Store all tools, equipment, and supplies before breaks or at the end of the workday.
  • Report all safety violations and hazardous conditions.
  • Follow established smoking regulations and report violations.
  • Report communicable or infectious diseases.
  • Follow established isolation precautions and procedures, washing hands before entering and after leaving isolation rooms.
  • Use and dispose of personal protective equipment according to established procedures.
  • Wear and use safety equipment when lifting or moving residents.
  • Report missing/illegible labels and SDSs.
  • Operate all equipment safely and use only necessary equipment and supplies.
  • Report defective equipment and inform supervisors of equipment and supply needs.
  • Review care plans daily and report changes in resident condition to the Nurse Supervisor/Charge Nurse.
  • Ensure notes reflect that the care plan is being followed.
  • Report suspected or known incidents of fraud.
  • Maintain confidentiality of resident care information.
  • Treat all residents with kindness, dignity, and respect.
  • Ensure nursing care is provided in privacy and knock before entering resident rooms.
  • Report all grievances and complaints to the Nurse Supervisor/Charge Nurse.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor resident refusal of treatment requests and report them to the supervisor.
