About The Position

The primary purpose of this job position is to provide each assigned resident with routine daily nursing care and services in accordance with the resident’s assessment and care plan, and as directed by supervisors. This role involves maintaining a safe and comfortable environment for residents, accurately documenting care provided, and communicating any changes in resident condition to the nursing supervisor.

Requirements

Must possess, as a minimum, a 10th-grade education.
Must be a licensed Certified Nursing Assistant in accordance with the laws of the state.
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
Ability to write reports, business correspondence, and procedure manuals.
Ability to effectively present information and respond to questions from managers and employees.
Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Occasionally lift and/or move up to 25 pounds.
Prolonged use of a desktop or laptop computer.
Regularly required to sit, stand, walk, talk, read, or hear.
Frequent use of all office-related equipment to include copier/scanner/fax, telephone, and calculator.
May be necessary to assist in the evacuation of residents during emergency situations.

Nice To Haves

CPR certification is preferred.

Responsibilities

Identify residents before administering treatments or serving meals using wristbands or photo cards.
Use only authorized abbreviations for recording information.
Report all changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
Record all entries on flow sheets, notes, and charts in an informative and descriptive manner.
Report all observed accidents and incidents as they occur.
Maintain resident confidentiality and report any suspected violations of protected health information disclosure.
Report any known or suspected unauthorized attempts to access the facility’s information system.
Perform only trained nursing care procedures.
Ensure resident rooms are ready for new admissions, including making beds and preparing admission kits.
Greet residents, escort them to their rooms, and introduce them to others.
Make residents comfortable and assist with packing or storing personal possessions and clothing.
Transport residents to new rooms or receiving areas and assist with loading/unloading from vehicles.
Create and maintain a warm, personal, and positive atmosphere, ensuring a calm environment.
Meet with nursing personnel to identify and correct problem areas and improve services.
Report all resident complaints and grievances.
Participate in and receive nursing reports.
Follow established policies concerning exposure to blood/body fluids.
Make beds and add extra covers as requested.
Measure and record residents' temperature, pulse, and respirations (TPRs), and weigh and measure residents as instructed.
Ensure residents unable to call for help are checked frequently.
Answer resident calls promptly and check routinely to ensure personal care needs are met.
Assist residents with identifying food arrangements and record food/fluid intake, reporting changes in eating habits.
Keep residents' water pitchers clean, filled, and within reach.
Perform after-meal care, including removing trays and cleaning residents' hands, face, and clothing.
Follow work assignments and schedules.
Cooperate with inter-departmental and facility personnel.
Notify the facility when late or absent.
Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
Assist residents with daily functions such as dental care, bathing, hair combing, and dressing/undressing.
Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
Assist residents in preparing for medical tests and examinations.
Assist with lifting, turning, moving, positioning, and transporting residents.
Perform restorative and rehabilitative procedures as instructed.
Check residents routinely to ensure personal care needs are met according to their wishes.
Observe and report pressure areas and skin breakdowns to prevent bedsores.
Report injuries of unknown source, including skin tears.
Observe disoriented and comatose residents and record/report data.
Perform special treatments as instructed.
Immediately notify the Nurse Supervisor/Charge Nurse of any resident leaving or missing from the facility.
Use only trained equipment and report defective equipment.
Maintain confidentiality of resident care information and report unauthorized disclosures.
Report all allegations of resident abuse and/or misappropriation of resident property.
Honor resident refusal of treatment requests and report them to the supervisor.
