About The Position
Rehab Without Walls, an affiliate of BrightSpring Health Services, is a revolutionary neurorehabilitation program that moves individuals outside the walls of institutional settings and into their natural home and community environment through a continuum of care. It is designed to significantly help people who have primarily experienced a brain injury, spinal cord injury, or stroke/CVA through accident or illness regain functional capabilities. By providing care in the home, community, workplace, or school, Rehab Without Walls promotes greater health through the recovery process, all with industry-leading outcomes that dramatically improve functional skills and independence for patients. It’s about the care team and the care plan, not the location where therapy takes place. Our innovative model of care combines multiple specialties that are not solely focused on what an individual can do today; rather we’re looking at sustainable long-term results months and years down the road. Specialized facilities that balance high-quality neurorehabilitation and homelike comfort. After a neurological injury or diagnosis, some individuals may require more therapy, supervision or physical care than can be provided at home. To help bridge the gap between the hospital setting and the home, Rehab Without Walls provides two types of residential care; Transitional and Supported Living.
Requirements
- Minimum age requirements is 18 years
- Valid driver's license, with satisfactory driving record, as defined by agency policy
- Certification, if required by state licensure as a CNA or Home Health Care Aide in the state in which the aide is to provide care
- Completion of a recognized home care aide training program or 6 months of recent experience as an aide
- Must be able to utilize proper body mechanics while lifting and may be expected to lift up 75 lbs.; must be able to sit, stand, and walk for periods of time and bend, stoop, push, pull, and reach
- Requires certification as a Nurse Assistant; functions may not exceed Standards of Practice as accorded by certification
Responsibilities
- Assists nurses by performing various basic care activities related to services essential to caring for the personal needs and comfort of patient
- Dresses, grooms, bathes, feeds, takes and records vital signs of patients
- Answers call lights, assists in toileting, oral care, nail care, catheter care, assists patients in wheel chairs, transports, turns bedridden patients, reports patient changes, and takes out trash
- Provides simple or uncomplicated patient care
- Cleans room and changes bed linen
- Takes and records temperature, pulse, and respiration rate
- Gives medication as directed by Physician or Nurse and makes notation of amount and time given
- Other duties as assigned
