Provides direct patient care under the supervision of a nurse. Works collaboratively with the care team. Greets and orients new patients to the hospital environment, room and unit routines. Creates a positive patient experience by focusing on what matters most to the patient and visitor/s. Fosters patient engagement by involving the patient and/or significant others in handoff to proactively meet patient needs and assure a safe environment through intentional rounding. Provides ongoing communication through regular updates to the patient-room care board. Promotes a Culture of Safety by mindfully integrating Error Prevention tools into daily routines. Contributes to the assessment of patients by collecting data for review by a nurse and reports abnormal findings. Documents patient progress toward goals and ensures documentation is accurate, complete, specific, and timely. Assists nursing staff to identify physical, psychosocial, age-specific, and cultural considerations of each patient. Works with the nursing staff to provide personal care according to organizational standards and policies.
