About The Position

The primary purpose of this position is to provide assigned residents with routine daily nursing care and services in accordance with their assessment and care plan, and as directed by supervisors. This role carries administrative authority, responsibility, and accountability for assigned duties. While efforts have been made to identify essential functions, these may not be exhaustive, and similar or related duties may be required.

Requirements

Must possess High School Diploma.
Must be a licensed Certified Nursing Assistant in accordance with state laws.
Must be able to read, write, speak, and understand the English language.
Must demonstrate knowledge and skills to provide care appropriate to the age-related needs of residents.
Must be a supportive team member and contribute to the team concept.
Must possess the ability to make independent decisions when circumstances warrant.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, and government agencies.
Must have the ability and willingness to work harmoniously with other personnel.
Must have patience, tact, a cheerful disposition, and enthusiasm.
Must be willing to handle difficult residents.

Must meet general health requirements, including medical and physical examination.
Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and sometimes hostile individuals.
Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet.
Must be able to push, pull, move, and/or carry at least 25 pounds a minimum distance of 50 feet.
May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
Identify residents using wristbands or photo ID before administering treatments or serving meals.
Record all entries on flow sheets, notes, and charts in an informative and descriptive manner using authorized abbreviations.
Report changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
Report all observed accidents and incidents on the shift they occur.
Maintain confidentiality of resident and facility information, and report any suspected violations.
Ensure resident rooms are prepared for admissions, including making beds and having admission kits ready.
Greet and escort new residents, introducing them to staff and other residents.
Assist residents with personal care functions including bathing, dressing, grooming, and oral hygiene.
Assist residents with bowel and bladder functions, maintaining intake and output records.
Assist residents with mobility, including walking and transferring between beds, chairs, and wheelchairs.
Perform restorative and rehabilitative procedures as instructed.
Change dressings, bandages, and binders as instructed.
Provide eye, ear, and skin care, and assist with compresses.
Assist in preparing residents for physical examinations and medical tests.
Weigh and measure residents, and record vital signs (TPRs).
Respond promptly to resident calls and check on residents who are unable to call for help.
Provide care for dying residents and perform post-mortem care as instructed.
Observe and report skin breakdowns and pressure areas to prevent bedsores.
Provide daily catheter and perineal care.
Perform Range of Motion exercises and turn bedfast residents at least every two hours.
Assist with physical and respiratory therapy as instructed.
Observe disoriented and comatose residents, recording and reporting data.
Prepare residents for meals, assist with feeding, and record food/fluid intake.
Keep resident water pitchers filled and serve snacks.
Perform after-meal care, including cleaning residents and their living space.
Attend and participate in scheduled training, educational classes, and orientation programs.
Wash hands before and after providing resident services.
Keep nurses' call system within reach and report missing residents immediately.
Follow established safety precautions and keep living areas clean and organized.
Clean, disinfect, and store resident care equipment after each use.
Perform routine housekeeping duties related to nursing care.
Report hazardous conditions, equipment, and safety violations.
Follow established smoking regulations and report violations.
Report communicable or infectious diseases and follow isolation precautions.
Use safety equipment, such as back braces and mechanical lifts, when moving residents.
Use equipment only after training and in a safe manner.
Report defective equipment and inform supervisors of equipment and supply needs.
Review care plans daily and report changes in resident condition to supervisors.
Maintain confidentiality of resident care information and report unauthorized disclosures.
Treat all residents with kindness, dignity, and respect, ensuring privacy during care.
Report all resident grievances, complaints, and allegations of abuse or misappropriation of property.
Honor resident refusal of treatment requests and report them to the supervisor.
