About The Position

We are seeking a compassionate and reliable Hospice Certified Nursing Assistant (CNA) to join Select Hospice. In this role, you will provide hands-on personal care to patients in their homes, focusing on comfort, dignity, and quality of life. You will support patients and their families during end-of-life care while working under the direction of a Registered Nurse (RN).

Requirements

• Current CNA license in the state which you are applying (required)
• Minimum 1 year of direct patient care experience (required)
• Listed on the Nurse Aide Registry with no findings of abuse or neglect

Nice To Haves

• High school diploma or equivalent (preferred)

Responsibilities

• Provide personal care including bathing, dressing, oral care, feeding, and elimination
• Assist with ambulation, positioning, transfers, and ADLs
• Measure and document vital signs and observations as directed
• Travel to and care for multiple hospice patients per day in their residences, following individualized plans of care
• Observe and report changes in patient condition or home safety concerns
• Follow RN care plans and agency policies
• Maintain patient confidentiality and HIPAA compliance
• Complete required trainings, meetings, and in-services
• Support patient comfort through socialization and emotional presence
