About The Position

Valeo Home Health & Hospice is seeking a compassionate and reliable Certified Nursing Assistant (CNA) to join their team. The CNA will provide hands-on personal care and support to both Home Health and Hospice patients in their homes, focusing on comfort, dignity, safety, and quality of life under the direction of the clinical team. This position starts as part-time with the potential to become full-time based on patient census and scheduling needs. The role requires flexibility in scheduling, dependability, and a passion for high-quality patient care.

Requirements

• Current CNA license in the state of Indiana (required)
• Minimum 1 year of direct patient care experience (required)
• Listed on the Nurse Aide Registry with no findings of abuse or neglect

Nice To Haves

• High school diploma or equivalent (preferred)

Responsibilities

• Provide personal care including bathing, dressing, oral care, feeding, and elimination
• Assist with ambulation, positioning, transfers, and ADLs
• Measure and document vital signs and observations as directed
• Travel to and care for multiple hospice patients per day in their residences, following individualized plans of care
• Observe and report changes in patient condition or home safety concerns
• Follow RN care plans and agency policies
• Maintain patient confidentiality and HIPAA compliance
• Complete required trainings, meetings, and in-services
• Support patient comfort through socialization and emotional presence
