About The Position

BrightStar Care of Plymouth is a growing home care agency seeking talented Certified Nursing Assistants (CNA) to provide in-home one-on-one personal care and homemaking for clients. This role has an immediate opening for overnights in Hopkins. We are a small agency providing personal care, homemaking, skilled services, and therapies for veterans in Western Hennepin and Eastern Wright Counties. Requirements

• High School Diploma or equivalent
• Must be Active on the MN Nursing Assistant Registry
• CPR Certified
• MUST BE ABLE TO TRAVEL up to 25 MILES within company's territory, as needed
• Must have Driver's License with insured, reliable transportation
• Must have a smart phone to utilize mobile apps for schedules and clocking in/out

Nice To Haves

• Minimum of one (1) year of experience as a Caregiver

Responsibilities

• Develop and promote a safe environment for quality client care through adherence to established policies, procedures, and standards
• Perform activities of daily living (ADL) as directed including, but not limited to: bathing, grooming, oral care, dressing, undressing, toileting, meal preparation and feeding
• Linen changes and light housekeeping
• Take and record vital signs as well as height and weight
• Monitoring of client's conditional changes, charting and reporting to the Director of Nursing
• Assist client in active and passive range of motion defined by the plan of care, assist client to doctor appointments and other outdoor activities as needed
• Other duties assigned by DON, including computer work in the office if necessary
