Requirements
- High school diploma or GED.
- Current certification as a Nursing Aide (CNA) in the state of TX.
- Current CPR certification.
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills.
- Reliable transportation and valid driver’s license.
Nice To Haves
- Minimum of 1 year of experience in hospice, home health, or long-term care.
Responsibilities
- Provide personal care services including bathing, dressing, grooming, toileting, and feeding.
- Assist patients with mobility, transfers, and positioning.
- Monitor and report changes in patient condition to the supervising nurse.
- Offer emotional support and companionship to patients and families.
- Maintain accurate documentation of care provided.
- Ensure patient comfort and safety in accordance with care plans.
- Adhere to infection control and safety procedures.
- Participate in interdisciplinary team meetings and contribute to care planning.
