About The Position

The primary purpose of this Certified Nursing Assistant (CNA) position is to provide assigned residents with routine daily nursing care and services in accordance with their assessment and care plan, and as directed by supervisors. The role involves administrative, admission/transfer/discharge, personnel, personal nursing care, special nursing care, food service, staff development, safety and sanitation, equipment and supply management, care plan adherence, budget awareness, and resident rights functions. The position operates within the nursing service area and requires the ability to handle frequent interruptions, communicate effectively, and work beyond normal hours, including weekends and holidays, as needed. The role may also involve being on-call during emergencies and requires the ability to cope with mental and emotional stress, as well as potential exposure to infectious diseases and hazardous chemicals.

Requirements

Must possess High School Diploma.
Must be a licensed Certified Nursing Assistant in accordance with laws of this state.
Must be able to read, write, speak, and understand the English language.
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
Must be a supportive team member, contribute to and be an example of team work and team concept.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
Must possess the ability and willingness to work harmoniously with other personnel.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
Must be able to relate information concerning a resident’s condition.
Must not pose a direct threat to the health and safety of other individuals in the workplace.
Must be able to move intermittently throughout the workday.
Must be able to speak and write the English language in an understandable manner.
Must be able to cope with the mental and emotional stress of the position.
Must be able to see and hear or use prosthetics that will enable these senses to function adequately.
Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
Identify residents using wristbands or photo ID before administering treatments or serving meals.
Record all entries on flow sheets, notes, and charts in an informative and descriptive manner using authorized abbreviations.
Report all changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
Report all observed accidents and incidents on the shift they occur.
Maintain confidentiality of resident and facility information and report any suspected violations.
Ensure resident rooms are prepared for admissions, including making beds and having admission kits ready.
Greet and escort residents to their rooms, introducing them to others as appropriate.
Assist residents with packing personal possessions during transfers or discharges.
Transport residents to new rooms or receiving areas, and assist with loading/unloading from vehicles.
Perform all assigned tasks in accordance with established policies and procedures and work schedules.
Cooperate with inter-departmental and facility personnel to ensure adequate nursing services.
Create and maintain a warm, positive, and calm environment.
Attend and participate in scheduled training, educational classes, and in-service programs.
Wash hands before and after providing services to residents.
Keep the nurses' call system within reach and check residents who cannot call for help frequently.
Assist residents with daily dental and mouth care, bathing, dressing, hair care, and nail care.
Shave male residents and keep female residents' hair clean shaven as instructed.
Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
Make beds (occupied and unoccupied) and ensure linens are tight.
Position bedfast residents correctly and comfortably.
Assist residents with bowel and bladder functions, maintaining intake and output records.
Keep incontinent residents clean and dry, and report bowel movement details.
Prepare and give enemas as instructed.
Collect specimens (urine, sputum, stools) as instructed.
Assist residents in preparing for medical tests, activities, and social programs.
Assist with lifting, turning, moving, positioning, and transporting residents.
Assist residents to walk with or without self-help devices.
Perform restorative and rehabilitative procedures as instructed.
Change dressings, bandages, and binders as instructed.
Provide eye and ear care, and assist with compresses.
Assist in preparing residents for physical examinations.
Weigh and measure residents, and record temperatures, pulse, and respirations (TPRs).
Answer resident calls promptly and assist with the care of dying residents, including post-mortem care.
Observe and report pressure areas and skin breakdowns to prevent bedsores.
Report injuries of unknown source, including skin tears.
Provide daily indwelling catheter care and perineal care.
Perform nonsterile vaginal irrigation/douches and assist with slings, elastic bandages, and binders.
Give tepid sponge baths and provide daily Range of Motion Exercises.
Turn bedfast residents at least every two hours.
Perform diabetic urine testing and provide physical and respiratory therapy as instructed.
Observe disoriented and comatose residents, recording and reporting data.
Turn all medications found in resident's room/possession over to the Nurse Supervisor/Charge Nurse.
Provide residents with Reality Orientation as instructed.
Monitor and report changes in room temperature, ventilation, or lighting.
Prepare residents for meals, including taking them to the bathroom, washing hands, and assisting with dining.
Serve food trays and assist with feeding as indicated.
Assist residents with identifying food arrangements, especially those with sight problems.
Record resident's food/fluid intake and report changes in eating habits.
Keep residents' water pitchers clean and filled, and serve between-meal and bedtime snacks.
Perform after-meal care, including cleaning residents and assisting with oral hygiene.
Check rooms for food articles, including unauthorized items.
Participate in scheduled training and educational classes to maintain certification.
Participate in scheduled orientation programs and activities.
Participate in facility in-service training programs.
Participate in in-service training programs prior to tasks involving potential exposure to blood/body fluids.
Follow established safety precautions and keep floors dry, reporting spills immediately.
Keep excess supplies and equipment off the floor and store in designated areas.
Clean wheelchairs, walkers, and resident care equipment after each use.
Perform routine housekeeping duties related to nursing care.
Store all tools, equipment, and supplies before breaks or at the end of the workday.
Report all hazardous conditions, equipment, and safety violations immediately.
Follow established smoking regulations and report violations.
Report any communicable or infectious disease to the Director of Nursing Services/Infection Control Coordinator.
Follow established isolation precautions and procedures, washing hands before entering and after leaving isolation rooms.
Wear and/or use safety equipment (e.g., back brace, mechanical lifts) when lifting or moving residents.
Report missing/illegible labels and SDSs to your supervisor.
Operate all equipment in a safe manner, using only necessary equipment and supplies.
Report defective equipment to the Nurse Supervisor/Charge Nurse.
Inform the Nurse Supervisor/Charge Nurse of equipment and supply needs.
Review care plans daily to identify changes in resident care routines.
Inform the Nurse Supervisor/Charge Nurse of resident condition changes for care plan updates.
Ensure notes reflect adherence to the care plan.
Report suspected or known incidents of fraud related to billings, cost reports, etc.
Maintain confidentiality of resident care information and report unauthorized disclosures.
Treat all residents fairly, with kindness, dignity, and respect.
Ensure all nursing care is provided in privacy and knock before entering resident rooms.
Report all grievances and complaints made by residents to the Nurse Supervisor/Charge Nurse.
Report all allegations of resident abuse and/or misappropriation of resident property.
Honor resident's refusal of treatment requests and report them to the supervisor.
