About The Position
Professional Case Management is hiring CNAs to provide in-home healthcare services to help those who have served our country. This role offers the opportunity to make a difference in clients' health, quality of life, and peace of mind through rewarding one-on-one patient relationships in the comfort of their homes. The position allows for flexible scheduling, enabling individuals to balance work with other commitments such as school, travel, volunteering, or family activities.
Requirements
- Active certification as a CNA in the state(s) of practice.
- Current CPR certification.
- Current TB test.
- Food Handler's card.

- Experience in Range of Motion assistance
- Experience in all activities of daily living including light duty housekeeping, meal preparation and patient hygiene
Responsibilities
- Provides personal care services under the direction of the RN/Case Manager.
- Performs services for clients as necessary to maintain their personal comfort.
- Engages in light duty housekeeping, meal preparation, and patient hygiene.
- Performs skilled transfers.
- Monitors and records vital signs.
- Completes timely and accurate documentation.
- Assists with range of motion exercises.
- Assists with all activities of daily living.
