About The Position

Vitality Hospice & Palliative Care is seeking a compassionate and dedicated Certified Nursing Assistant (CNA) to join our growing team. This is an opportunity to work for a clinician-owned and operated hospice that truly values both its patients and its employees. This is a field-based position with travel required, and occasional evening/weekend overtime is available. The role involves providing essential hands-on care and support under the supervision of a Registered Nurse Case Manager, including assisting with mobility, personal care, offering companionship, performing light housekeeping, and documenting care accurately. You will be an integral member of our interdisciplinary team focused on clinical excellence and patient-centered care.

Requirements

• Active Certified Nursing Assistant (CNA) certification
• Valid driver’s license, reliable vehicle, and current auto insurance
• Compassionate, reliable, and self-directed
• Strong communication and time-management skills
• Ability to work independently and follow care plans
• Commitment to serving patients with chronic, serious, or terminal illnesses

Responsibilities

• Assisting with mobility, transfers, and ambulation
• Providing personal care (bathing, grooming, dressing, feeding)
• Offering companionship and emotional support
• Performing light housekeeping tasks
• Documenting care accurately and in a timely manner
• Supporting a culture of compassion, quality, and respect
