About The Position

The primary purpose of this Certified Nursing Assistant (CNA) position is to provide assigned residents with routine daily nursing care and services in accordance with their assessment and care plan, as directed by supervisors. The role involves administrative, admission/transfer/discharge, personnel, personal nursing care, special nursing care, food service, staff development, safety and sanitation, equipment and supply, care plan, budget and planning, and resident rights functions. The CNA is expected to maintain a high standard of resident care, uphold facility policies, and contribute to a positive and safe environment.

Requirements

Must possess High School Diploma.
Must be a licensed Certified Nursing Assistant in accordance with laws of this state.
Must be able to read, write, speak, and understand the English language.
Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
Must be a supportive team member, contribute to and be an example of team work and team concept.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
Must possess the ability and willingness to work harmoniously with other personnel.
Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
Must be able to relate information concerning a resident’s condition.
Must not pose a direct threat to the health and safety of other individuals in the workplace.
Must be able to move intermittently throughout the workday.
Must be able to speak and write the English language in an understandable manner.
Must be able to cope with the mental and emotional stress of the position.
Must be able to see and hear or use prosthetics that will enable these senses to function adequately.
Must function independently and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.
Must meet the general health requirements set forth by the policies of this facility, which include a medical and physical examination.
Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility.
Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push, pull, move, and/or carry such weight a minimum distance of 50 feet.
May be necessary to assist in the evacuation of residents during emergency situations.

Responsibilities

Provide routine daily nursing care and services to assigned residents according to their assessment and care plan.
Identify residents using wristbands or photo ID before administering treatments or serving meals.
Record resident information accurately on flow sheets, notes, and charts using authorized abbreviations.
Report any changes in resident condition to the Nurse Supervisor/Charge Nurse promptly.
Report all observed accidents and incidents on the shift they occur.
Maintain confidentiality of resident and facility information, including protected health information.
Ensure resident rooms are prepared for admissions.
Assist residents with admissions, transfers, and discharges, including packing personal possessions and transporting them.
Perform all assigned tasks in accordance with established policies and procedures.
Cooperate with inter-departmental and facility personnel to ensure adequate nursing services.
Create and maintain a warm, positive, and calm environment.
Participate in nursing report upon reporting for duty.
Assist residents with daily personal hygiene, including dental care, bathing, dressing, hair care, and nail care.
Shave male residents and keep female residents’ facial hair clean-shaven as instructed.
Keep residents dry and change linens as needed.
Make beds (occupied and unoccupied) and ensure linens are free of wrinkles.
Position bedfast residents correctly and comfortably.
Assist residents with bowel and bladder functions, maintaining intake and output records.
Keep incontinent residents clean and dry.
Prepare and give enemas, reporting results as instructed.
Collect specimens (urine, sputum, stools) as instructed.
Assist residents in preparing for medical tests, appointments, and social programs.
Assist with lifting, turning, moving, positioning, and transporting residents.
Assist residents to walk with or without self-help devices.
Perform restorative and rehabilitative procedures as instructed.
Change dressings, bandages, and binders as instructed.
Provide eye and ear care, and assist with compresses.
Assist in preparing residents for physical examinations.
Weigh and measure residents, and record temperatures, pulse, and respirations (TPRs).
Respond promptly to resident calls and check on residents who cannot call for help.
Assist with the care of dying residents and provide post-mortem care.
Observe and report pressure areas and skin breakdowns to prevent bedsores.
Report injuries of unknown source, including skin tears.
Provide daily indwelling catheter care and perineal care.
Perform nonsterile vaginal irrigation/douches and tepid sponge baths.
Provide daily Range of Motion Exercises.
Turn bedfast residents at least every two hours.
Perform diabetic urine testing and special treatments as instructed.
Observe disoriented and comatose residents, recording and reporting data.
Turn all medications found in resident’s room/possession over to the Nurse Supervisor/Charge Nurse.
Provide Reality Orientation as instructed.
Monitor and report changes in room temperature, ventilation, and lighting.
Prepare residents for meals, assist with feeding, and supervise dining room activities.
Record resident food/fluid intake and report changes in eating habits.
Keep residents’ water pitchers clean and filled with fresh water.
Serve between-meal and bedtime snacks.
Perform after-meal care, including cleaning residents and their personal items.
Check rooms for unauthorized food items.
Attend and participate in scheduled training, educational classes, and in-service programs.
Wash hands before and after providing services to residents.
Keep the nurses’ call system within reach and report any resident leaving/missing from the facility.
Follow established safety precautions, keep floors dry, and report spills immediately.
Store supplies and equipment properly and clean resident care equipment after each use.
Perform routine housekeeping duties related to nursing care.
Report all hazardous conditions, equipment, and safety violations.
Follow established smoking regulations and report violations.
Report any communicable or infectious disease to the Director of Nursing Services and/or Infection Control Coordinator.
Follow established isolation precautions and procedures, including hand washing and proper use of personal protective equipment.
Wear and/or use safety equipment when lifting or moving residents.
Report missing/illegible labels and SDSs to your supervisor.
Operate all equipment safely and use only necessary equipment and supplies.
Report defective equipment and inform supervisors of equipment and supply needs.
Review care plans daily and report any changes in resident condition to the Nurse Supervisor/Charge Nurse.
Ensure notes reflect that the care plan is being followed.
Report suspected or known incidents of fraud.
Maintain confidentiality of resident care information and report unauthorized disclosures.
Treat all residents fairly, with kindness, dignity, and respect.
Ensure all nursing care is provided in privacy and knock before entering resident rooms.
Report all grievances and complaints made by residents.
Report all allegations of resident abuse and/or misappropriation of resident property.
Honor resident refusal of treatment requests and report them to the supervisor.
