About The Position

The purpose of the CNA position is to provide personal cares, delegated nursing services, and light housekeeping services to maintain the client's physical and emotional well-being. The CNA will work with hospice patients to provide personal care services, nurse-delegated tasks, and light housekeeping services as directed by the patient's RN Case Manager and their established plan of care.

Requirements

• Minnesota CNA Certification required
• Must be 18 years of age or older
• Valid driver's license, auto insurance and reliable transportation required
• The ability to pass a background check
• Strong verbal and written communication skills
• Basic computer skills

Nice To Haves

• Previous hospice experience is strongly preferred
• At least 1 year of previous hospice experience, preferred

Responsibilities

• Provide personal cares
• Provide delegated nursing services
• Provide light housekeeping services
• Work with hospice patients to provide personal care services
• Perform nurse-delegated tasks
• Perform light housekeeping services as directed by the patient's RN Case Manager and their established plan of care
