The Certified Nursing Assistant (CNA) is responsible for providing compassionate, high-quality care to hospice patients in the home setting, addressing the physical, psychological, social, cultural, spiritual, and developmental needs of each individual and their family. Assist the hospice clinical team in delivering comfort-focused care, including simple treatments as directed, observation, and accurate documentation in alignment with hospice standards and care plans. The CNA functions as a key member of the interdisciplinary hospice team, supporting continuity of care through timely communication and collaboration with the RN Case Manager and other clinical staff. Maintains a safe, clean, and supportive environment for patients in their home, promoting dignity, comfort, and quality of life at end of life.
