About The Position

The Certified Nursing Assistant (CNA) is responsible for providing compassionate, high-quality care to hospice patients in the home setting, addressing the physical, psychological, social, cultural, spiritual, and developmental needs of each individual and their family. Assist the hospice clinical team in delivering comfort-focused care, including simple treatments as directed, observation, and accurate documentation in alignment with hospice standards and care plans. The CNA functions as a key member of the interdisciplinary hospice team, supporting continuity of care through timely communication and collaboration with the RN Case Manager and other clinical staff. Maintains a safe, clean, and supportive environment for patients in their home, promoting dignity, comfort, and quality of life at end of life. Requirements

  • Completion of a Certified Nursing Aide (CNA) training program and State issued certification as Nurse Aide.
  • CPR certification
  • At least one (1) year of practical field experience.
  • Has familiarity with medical equipment, patient charts, medical terminology, and documentation procedures.
  • Familiarity with Microsoft applications and Google, and knowledge in EMRs.

Nice To Haves

  • Experience in Hospice, Home Health, or a Skilled Nursing Facility.

Responsibilities

  • Assist with daily activities of living including: general hygiene, shampoos, nail care, shaving and dressing.
  • Document all personal care given at the end of each shift.
  • Perform skin care activities to include turning every two hours and proper positioning.
  • Transfer bed to chair, chair to toilet and chair to walker using proper body mechanics and techniques or transfer devices such as a Hoyer lift.
  • Perform incontinent care to include pericare, bowel and bladder training and toileting.
  • Serve meals, feed residents and encourage use of adaptive devices when ordered.
  • Assist with reminders to take medications that are ordinarily self-administered.
  • Manage Foley catheters including proper positioning of tubing and drainage bag, emptying of drainage bag and catheter care.
  • Take vital signs to include proper use of thermometer, rectal, oral, ear and other.
  • Practice appropriate infection control - to include use of gloves, hand washing and universal precautions.
  • Perform care of transitioning residents and post-mortem care.
  • Report any unusual symptoms or change in resident's condition or unsafe conditions, and respond according to protocol.
  • Read and interpret care plans and use approaches established to reach goals.
  • All other duties as assigned.
